Car jerks accelerating at low speed

1996 Nissan Sentra Jerks while accelerating to 10 mph, after 10 mph it stops jerking and runs normal

The problem started when I was driving at 60mph and it started to feel like it was driving on 2 rather than on D, I noticed the distributor was leaking so I went ahead and replaced it myself this time. The car starts like normal, everything sounds good but I noticed that it has a bit of a jerk motion when I accelerate it right at 10mph it jerks and it only happens on D and on 2 but it does not jerk at all when I have it on 1. It's an automatic transmission, the fluid is clean.

How many miles? After 28 years it would be no surprise if the transmission is wearing out.
