I’m looking at a 2007-2009 Lexus LS 460 to buy as a second car. I know they have a 4.6L V8. Is it a pretty reliable car overall? Also, must I burn premium fuel in the car? Or can I at least alternate between mid grade and regular?
Any Lexus LS 460 in my price range will have around 150,000 miles on it.

They are very expensive in maintenance. Consider the LS430 model instead of you like this type of cars.
Just look up on Youtube something like "How much have I spent on my Lexus LS460" and Youtubers who specialise in Toyota and Lexus maintenance and they will tell you the high cost and typical parts that fail on these models.
Supsension seems to be a real problem area. I also vaguely remember another mechanic talking about plastic/rubber parts Lexus put inside the V of the engine that become brittle when maintenance is needed.
And there was another video where a known mechanic said he coldn't repace a part on the car because it was locked behind software he didn't have the licence for. So if you went ahead, you might have to hunt around for a specialist mechanic first.
