Good day Scotty. I live in Rhode Island and I have an opportunity to buy a one owner 2003 Silverado 1500. 5.3 v8 4x4 with 44,000 original miles. The truck is in phenomenal condition. Glove box full of paperwork. It hasn’t been used much in the past year due to owner passing away. What should I be concerned with on a old truck with very little use. Thank you very much for your help.
Almost anything could go wrong on a 20-year-old vehicle. (Age takes its toll as well as mileage. That low mileage should be verified.) Have a good mechanic give it a thorough pre-purchase inspection.
This should give you an idea of some specific things to look out for:

Go for it. That should be an awesome pre-bankruptcy truck for the right price. Good solid engine. Make sure you put money aside though. It's going to need some love for the aging parts after all those years. Be nice to the transmission.
Since it's a New England vehicle check the frame carefully for RUST.
