Clacking brakes

I had the pads and rotors replaced on my father's 2011 Toyota Venza and immediately after I noticed a clunking/clacking sound coming from them when driving low speed, usually in parking lots when backing up then going forward, etcand first starting out. OEM pads, Bosch rotors.  I had the mechanic who put them on inspect them and flush the brakes - he said they looked fine and he put some grease on... something. I believe it helped for a short time but now back to clack clack clack.  What's going on here?

My bet was at the installation of the brakes were wrong. Clicking or rattling sounds at low speed if the brakes were just changed is more than likely to indicate that the hardware or bracketry used with the pads are not secured properly or even wrong. That would be my guess. If it do be a new or intern mechanic give the guy a little slack. It's the shop's fault for not watching over him to make sure the job was good.
