Have a 200,000km service coming up.. the book says to change the diff oil... but in brackets (automatic)... the car is a manual toyota camry from 1995 with 198,888km..
DO I need to do this? Is it expensive?

It sounds like a lot but I have no idea what labor rates or going prices for those services are like in Australia. (Heck, I don't even know how you guys manage not to fall off the bottom of the earth.) You could shop around a bit to get competitive quotes. As in the U.S. you would want to avoid dealers and chain shops in favor of a good independent mechanic.
Changing the engine oil and transmission lube should be an easy enough job to tackle if you're inclined to do it yourself. If you have a 4-cylinder engine changing the plugs would also be very easy. (If you have a V6 engine the rear plugs are tough to get to but a quick search says it can be done without removing the intake.)

Generically, yes. However you need to check the specifications for the lube required in your particular vehicle. That should be in your owner's manual.
Why wouldn't you want to change out old differential oil, or for that matter the manual transmission lube?
there also Manual tranmission oil to change too. IS THE diff. oil change same? OR will both the manual trans. oil and the diff oil both need to be changed?

They are probably the same on your Camry and for that matter virtually any front-drive car equipped with a manual transaxle. (Most automatics as well.) I was thinking in terms of AWD cars where there are center and rear differentials that would need service as well.
