This is a really good question, do turbochargers lose efficiency as they age? Even if it were making a certain psi, it won't sound as good as a new one right?

Not in any noticeable way. The performance of the engine will decrease as all of the parts wear, but a good turbo can run for a LONG time.
20 psi on a 5 year old turbo sounds like 20 psi on a brand new one. If it doesn't, there is an issue with that old turbo.
