I have a 2006 Ford f150 fx4 5.4
I had the belts and pulleys changed awhile back and around 6 months later the serpentine belt fell off I got it back on it was fine for a week then it fell off again my mechanic replaced all the pulleys and belt and around 3 weeks later the belt completely snapped and broke off with having new pulleys and a belt the truck sat a week while I was on vacation came back and started it up and it made a boom sound when it broke my mechanic put a new tensioner and belt on but the pulleys are still making a squeaky sound
any idea what could be causing the belt to keep coming off I’m preying it won’t happen again

I can't see your engine bay, so anything I say is a wild guess.
One of your accessories could be seizing up. Alternator, AC compressor, etc.

A photo probably won't reveal anything.
Maybe unplugging the AC compressor will disable it. Not too sure.
You can't unplug the alternator, unfortunately, but you could try turning by hand and feeling for resistance.
Try it with a heavy electrical load turned on like headlights, blower, etc.
