My wife has 2025 Toyota Rav4, unfortunately cought a screw in one tire, very close to the side. Most people are saying that should be replaced... My question is, should I replace one tire or both if the milage is 4900 miles in All Wheel Drive...??

In general all 4 tires on an all-wheel-drive vehicle need to be replaced together if one is damaged beyond repair. The specific information should be in your owner's manual.
The tires all have to be within a certain diameter or circumference of each other to avoid undue stress on the all-wheel-drive system. They should also all be the same brand and type as well as preferably all from the same manufacturing lot.

Can't answer that 100% for you, but with such low miles it's probably fine. The only way you can know for sure is to measure the tire diameter (very accurately!). General rule of thumb is the tire circumferences should be within about a quarter of an inch. That works to about a 1/16 inch of tread wear.
