I have a 1997 Dodge Ram Van with a 318 engine with 228,000 miles. On start up I get a very loud noise from the pulleys. I have had the tensioner, idler pulley, alternator and belt changed but the very loud noise continues. Any help would be appreciated. I have a video of the noise but couldn’t see how to attach it. Thank you.

if you take the belt off, does the noise go away?

It's really hard to suggest cures remotely, but I would suggest timing chain, water pump bearing.
Never thought of timing chain. Thanks for the suggestion. Engine runs and idles smooth and silent though. I have replaced the water pump several times but not recently.
I think I have same problem. I'm Hearing a metallic scraping sound similar to what was in your video. At 1st I thought it was the water pump changed that didn't help Then I change the Idler pulley and even the harmonic balancer still hear Scraping noise. IT Diving me mad 😠 Any helpful tips or what I should be looking for?
