I get my tires rotated every oil change (5k miles), should I have them balanced every time? I have no problems with my tires now. Thanks: Dave
No problems, no worry. I base balancing on how it's driven/used. A daily commuter driven on roads under the law and properly balanced when installed, should never have to be balanced again.
Sport and off road, more uneven tire wear and chance to knock weights off, so as needed by noticing during driving. Of course these can happen also on a daily road car (hit curb, rock or something knock weight off).

No. If you rotate tires often - like you indicated, you will get substantial increase in life of your tires and balancing is pretty well redundant, … unless a balance weight gets unglued or something like that.
