Cars these days use timing belt or chain, what about timing gears that never been put on modern cars?

gears are expensive, they have backlash, and they're noisy.

Gear drives are noisy and are expensive for overhead cams so they're not used in modern mass-produced cars. They were mostly found in pushrod engines back in the day where the cam was close enough to the crank that only 2 gears were needed, like early Subaru boxer engines.
