By "worst" do you mean least reliable/quickest to fall apart, or one that generates the most cussing and skinned knuckles while working on it?
For the first category there are quite a few candidates but it would be tough to beat the utter and complete crapulence of the Chevrolet Vega.
In the latter category, for a good time try changing the valve cover gasket on a six-cylinder AMC Pacer.

Worst car I have personally ever seen was probably the PT Cruiser. I still some of them with over 150k and I wonder how many engines has it went through.
