1997 GMC Yukon P1406 Will Not Clear

Hello,

My 1997 GMC Yukon has had the MAF cleaned, the IAT replaced, and the EGR replaced.

The P1405 code still comes and goes.  The ruck drives fine at speeds above 25-27 mph.  When the code is not on it idles a little rough but it does seem to run fine from 0 on up to 70mph.  When the code is on (checked via bluedriver scan tool), the truck has a bit of rough idle and seems to run a bit rough up to 25-27mph.  But then runs fine at higher speeds.

What could be causing this code to come and go?  How do I get rid of it once and for all?
