Thinking of purchasing this truck from a buddy he's the original owner - wondering with 153,000 KM 0r 96,000 Miles do you recommend changing the automatic transmission fluid as it was never done in 23 years??

Might be all right if it's not too much money and a mechanic says it's OK. (Have it inspected before purchase.) How much longer do you expect to get out of a truck that's already 23 years old? Even with relatively low mileage there will be age-related issues to deal with. Pay particular attention to rust problems if you live in an area with salted winter roads.
A vehicle that old is most feasible to own if you do your own repairs. If you need to take the thing to a mechanic every time something goes wrong it could quickly nickel and dime you to death, and that's assuming no major problems.
