Hi Scotty. I have a 2001 Toyota Camry 2.2L engine with 200,000 miles on it. Idle is smooth when it's in park or neutral but when I put it in gear, either drive or reverse, it would start surging or the idle gets rough. What could be the problem? Thank you.

Odds are that you don't get correct air/fuel mixture.
Change the air filter, clean the MAF sensor, Change the fuel filter and then pressure test the fuel pump.

Does the vehicle lack power in addition to the surge? If so, you may want to check for a clogged catalytic converter.
