I have a 2003 Honda Pilot with 325,000 miles, Automatic Transmission, the car jerks a little only between 2nd and 3rd gears and shifts roughly. This started a few months ago but now it's more frequent, usually after I drive the car for a while, is there anything I can do to fix it? or is the transmission totally gone?
I bought the car last July, changed the ATF with Honda genuine ATF, it was shifting well up until the beginning of 2023 approximately.

Your transmission is probably dying. It's to be expected at that age and mileage. (Honda automatics of that vintage are known to be on the weak side.)
If you're lucky it might just be something like a shift solenoid but it would take a transmission expert with a fancy bidirectional scan tool to make that determination.
