Hey scotty, I recently had the rear main seal changed on my 2006 sienna with the 3.3L 3mz-fe engine. Since then I've been hearing this weird groaning noise at idle, but it goes away when after you start accelerating. Its very apparent in Reverse and Drive but you cant still hear it no matter the gear. The car still runs as smooth as it used to, is this detrimental?

depends what it is.
Go back to the shop and tell them what happened.
Or use your own eyeballs and check. It might just be a loose heat shield or somebody's screwdriver fell on the exhaust.

Take it back like MMJoe said and have them check the motor and trans mounts to make sure they were properly tightened.
