My Tacoma LTFT runs at 2.3 with no load and at 4.7 with some load. The average gas consumption is a bit high: 17 mi/gal mix highway and city driving.
It has 150K miles well taken care of. Very conservative driving.
I have replaced all spark plugs and sensors there are with Denso/Toyota, plus I used Bernie ATS Fuel cleaner to no avail.
What else should I be looking for?
Could the fuel injectors be going out or too dirty?

Bernie's cleaner should have cleaned the injectors. An older vehicle will often have fuel trims similar to yours, which aren't really 'bad'. Try Scotty's advice here: Make Your Car Run Better with a Little Spray Cleaner
Well for some reason it's adding too much fuel. I would start by using my friend Bernie from Albuquerque's ATS fuel cleaner. Just Google that. That often will help
