Certainly sounds like your CVT is going out.
You can try some CVT Transmission Fix but I doubt it would really help.
You could gamble and try to stick in a refurbished or junkyard CVT but that vehicle isn't really worth much, so I wouldn't advise anyone to put money into one.
But if you want, you can try to use some Bar's CVT juice in there to get a little more life out of it.
I unfortunately have to agree with what has been said. It sounds like that CVT is on it's way out.
I'd start looking for a replacement vehicle.
The transmission is going out, find a new vehicle, this time not a Nissan.
