My cousin has a 2011 Subaru outback wagon with 182,000 mi. Recently the engine has developed a sound that I believe to be rod knock. It's also been running a little lean, and seems to slightly cut out as if a cylinder is completely gone sometimes under acceleration. What might be the issue here?

Any codes at all? I'd ask them if they drove through water recently.
Also, have you had a look at the oil? If the head gasket is going & just enough coolant got into the cylinder that could account for the sound/loss of power.
