So I have a 2014 honda accord ex with an automatic transmission. The car just passed 41k miles on it and grinds on a cold engine. Is this the VCT actuator? Why at 41k?

You never said what engine you have. 4 cyl.has a chain, V6 has a belt. If you have the 4 cyl it could be timing chain noise.
You’re right I forgot to mention that it is a 2.4 L
It is possibly a failed timing chain tensioner. Are you sure about the mileage in this 8 year old car? Poor maintenance regarding oil and filter changes can cause premature chain tensioner failure.
Could also be carbon build up from the new GDI system introduced in the 2013 2.4L engine.
yes I am sure about the mileage I bought it a few years ago with 12k miles. A 90 year old women owned it before me. The oil is changed every 5k miles with full synthetic oil I do all my own maintenance and make sure it’s done right. So I know it’s not poor maintenance but not sure why it does this
