What Carfax can't show you is whether it was wrecked or flooded and fixed off the books.

No way to tell unless you know someone who has a working crystal ball. However the majority of vehicles do not make it to 400,000 miles or more without requiring major repairs.

depends on the condition of the engine, drivetrain, suspension, frame, etc and how well it was looked after. Don't buy anything without a mechanical inspection.
