Bought a 2019 Lexus, my wife does not like when she starts off from a start that take off too quickly, how can you change the Transmission to have a less aggressive start. ( can Transmission or accelerator be changed with a OBD Diagnostic Tool ?)

Your wife is pushing too fast on the accelerator.
I suggest your wife learn to adapt to the throttle response of the vehicle.
It's theoretically possible to alter the throttle response parameters, but not at the dealer level or below.
Manufacturers settings for best performance, fuel economy, reliability and longevity for vehicle operation within a market region are hard-coded (burned) into chips.
Replacing a chip is not a viable option because such chips are (most commonly) built into substrates of the applicable control unit circuitry, and such "circuit boards" can easily have 10, 20, or more "layers" of circuitry present on a single physical control component.
In the drive mode selection, choose ECO. If it doesn’t give you the result you want, drive more conservatively.
Have it in ECO drive mode.
She's driving it like a golf cart. Gas pedal all the way down or up. Not much in between. My wife drives like this. She has a 2017 RX350. She would never complain it had too much acceleration though.
