My dad has a 93 Chevy Silverado with about 200k miles. It runs good and all, but when it hits second gear when accelerating, it is not a smooth gear shift and it makes the truck jerk a bit. It is only second gear though, the rest of the gears shift very good. What could this mean?

so you mean the 1-2 shift
This is normal for transmissions as they age. (200k is quite a bit)
Especially GM trannies I find the 1-2 shift on all of them is a bit rough. Probably because the gear ratios are far apart.

If I were you, I would live with it till the wheels fall off.
