Ford e150 hesitates when accelerating

Hey scotty. My 1998 ford e150 (354k kms 4.2L v6) hesistates very badly and sputters when im trying to accelerate up a very small hill. What can i do?

Fuel filter, injectors, fuel pump, air filter, etc.
