Assuming there is sufficient transmission fluid your transmission is probably wearing out. Has it been serviced regularly?

You didn't mention the mileage. If you've never changed the transmission fluid, it's most likely wearing out. My old Pontiac has a minor transmission leak. I need to add around a pint to a quart every 6 months. When it's time to top up, I notice the transmission hesitates for a second, and then it goes back into gear.
