Since it got truncated and messed up you might want to put your question here again as a response.
First, check all clutch adjustments. If they are OK, you probably need a new clutch package (clutch, pressure plate, throwout bearing and pilot bearing. Check the operation of the master and slave cylinders as well and fluid level.
