2001 honda civic ex, 1.7 vtec, d17a2 engine.

Gas mileage down 5 mpg, car starts sounding like fast idle on a carbureted vehicle. Scanned trouble codes initially were P0135, Bank 1, Sensor 1 then P0141, bank 1 sensor 2. I checked connections (b1s1 loose), reset pcm, checked underdash fuse #4.
Now I am getting only "P0141 Bank 1, Sensor 1". In all my research I thought the 0141 code was for sensor 2?? What did I do wrong? How do I fix it?

Where does it say Bank 1, Sensor 1? P0135 refers to Bank 1, Sensor 1.
P141 is O2 Sensor Heater Circuit Malfunction (Bank 1, Sensor 2).

That's the weird part. My Autel 301 Scanner is what is giving that code readout now.
Thank you, I have been through all that. What I am asking is WHY is my scanner telling me that? What would cause that/ Is there any way that the sensor KNOWS it's a downstream sensor other than the circuit it is on?
What could I have done wrong?
I used to have the P0141 code as bank 1 sensor 2, along with the P0135 code for bank 1 sensor 1. Now all i have is P0141 for bank 1 sensor 1 on my Scanner.
