Noise or leaking or both are the usual symptoms and the pump can even lock up if you let the wear go too far.  On some pumps the impeller can come loose or corrode, causing overheating. If your water pump is driven by the timing belt it is best to replace it when the belt is replaced, or go ahead and replace both if you don't know how long they've been on the car. On the other hand, if the pump is driven by a fan belt and there are no symptoms I wouldn't worry about it.
