Hello I have a 2000 Silverado 159,000 miles, 4.8 v8 4speed auto. At idle it sounds like engine rpm goes the settles down,and does it continuously but I've been using premium fuel nothing idles even, I do not know if I should be concerned or not. Runs good on highway makes good power, no check engine light

Clean throttle body and MAF sensor, check for vacuum leaks, perform a diagnostic scan.
Thank you, I also have a question could it also be the different octane of fuel? Ever since I have been refueling with premium the truck idles even no issues.
Seems unlikely to just be the octane.

premium fuel is like a crutch. It's just hiding the engine problems.
