I have a 2012 Honda Civic Si (manual transmission) with 224k miles. When going at low speeds (below 25mph) the car jerks when trying to maintain my speed. Whenever I try to use the smallest amount of throttle to maintain my speed or even take my foot off the gas pedal completely, It tends to jerk pretty hard. At higher speeds, it's not an issue. I've had the car for about 4k miles and I don't know how well the car was maintained. I did have a P2127 code about 800 miles ago but it hasn't come back since I cleared it then. I've cleaned the MAF sensor and throttle body but it's still jerking. What could it be?

What could it be?
At that age and mileage it could be almost anything. How old are air and fuel filters? Spark plugs? Is fuel pressure OK? Are fuel injectors clogged? What do fuel trims look like? Have you tested the accelerator sensor?
I do not know when or if the fuel filter and spark plugs have been changed. The air filter looks to be in good shape. After 20min drive short fuel trim was 5% and 7% for long fuel trim. I have not tested the pedal or the fuel pressure.
