Occasional 1999 Toyota XL rough Idle

Hi Scotty:

What could cause my 1999 Toyota XL with 154,000 miles to have an occasional rough idle?

Most of the time otherwise, the car runs smoothly.

What can I do to fix the occasional rough idle?
