Car is a 2004 Mercury Grand Marquis GS, 102.3K currently. Bought it last December at 99.8K. Changed the air filter then, snd it’s been nearly 3K since. I’ve heard people say to change it every 10-15K miles, but others said change it once a year. It still looks okay.

Change it when it's dirty.
If you drive dirt roads everyday, you'll be changing it several times per year.
Pull it out and hold it up to a strong light to see how dirty it is.

I would. Scotty Kilmer takes them out and says you need to see daylight through them in his videos. Can't remember how often he recommends changing though.

Those are both good intervals, whichever comes first.
