I have a 2005 Toyota Sienna with 100,000 miles. Runs great and no overheating issues. Should I change the thermostat as a preventive maintenance issue being that it's old or if it isn't broke don't fix?

If it’s not broke, don’t fix it. I would leave it.

And the radiator cap too. See owner's manual for coolant change intervals.

Whenever you decide to change the coolant, you can change the thermostat as well.
