Purchased new 06 Honda Pilot 124,000 miles, runs great, neverva problem, just tires, brakes, battery and regular oil changes. Do I need to replace the timing belt.

If you have an interference engine, then yes.

If it’s never been replaced, absolutely yes, do it now......your overdue....

If it hasn't been changed ever then yes definitely change it very soon and try to change it every 100k miles or so. Honda recommended that I change mine at 100k miles for the needed maintenance.

That’s a interference engine, hope and pray it doesn’t break, because if it does your up shits creek.....

Do it, check your owners manual for the intervals, but you are way past the recommended time I'd do it before it goes out and the engines blows up.
