variable valve timing cam actuator damage

Greetings - '08 Toyota Matrix, 1zzfe 1.8L with startup clatter until oil pressure is established.  I found one of the four vanes of the actuator internal moving core snapped clean off and lodged in sideways.   What causes this?   Should I be looking upstream for something that makes this happen?
