Hi, I have a 2004 Volvo xc70 with 250k miles, when I brake then accelerating the car jerks forward (feels like it doesnt engage right away) and you can hear a clunk. They motor mounts and cv axles were recently replaced.

Automatic or manual transmission? If automatic when was the last time the fluid and filter (if so equipped) were changes?
Its an automatic transmission and the transmission fluid was never changed.
I would say at 250K miles if the fluid has never been changed that's at least part of the problem right there.
