I wanna know how to be able to tell if a mechanic is pulling a fast one on me or not and I’m sure there is a couple of questions we could ask to determine based of their response.
Ask him to check your muffler bearings or blinker fluid.

I think I need head gasket changed. Can you do it for me? An honest mechanic will question you as to why, what are your symptoms, why do you think that etc... A dishonest one will go sure, we can do that.
one question sounds very unlikely.
A better idea is to take it in for routine oil change + checkup and see what they say.
I don't think there is a silver bullet question to ask. I would just have a conversation and get a feel for the guy/gal.
They will diagnosis first and then provide an iron-clad guarantee that their work will fix the problem, no maybes about it.
