Hey I have a 1999 4runner V6 Auto that frequently but not every time misfires, shaking and smell of fuel from exhaust when cold. Once the engine heats up it stops.
The only code it has is for catalytic converter below efficiency threshold bank 1 but I believe that this was caused by the misfire.
oem spark plugs, o2 sensors and maf sensor are less than 1 year old.

Try some fuel injection cleaner.
