Hesitation while accelerating

Hi all, New guy here with a question.

My mother has a 2003 ES300 with 145k miles.  She is the original owner.  She doesn't drive much.  Maybe 4k miles per year. 

Just yesterday I was taking her out on some errands with her vehicle and have noticed a hesitation upon acceleration through all the gears.

There is no check engine light on.  No codes have been scanned for either. 

When applying moderate acceleration the vehicle has a steady "surging" throughout its range.  Its not a dramatic bucking motion, just a very persistent hesitation. 

Best way to describe it is, smooth power then a mild loss, smooth power again then mild loss.  Its extremely consistent and rhythmic. 

I was researching and was thinking of doing a scan first for any stored old codes.

Perhaps it could be a bad MAF?

A bad Throttle position sensor? (did this year even have one?)

Cracks in the air box boot? 

Throttle body cleaned?

Im just spit balling ideas here.  Anyone have a good step by step troubleshooting process for this?

That thing is a couple of decades old, it could be almost anything. Scanning for codes and live data would be a good start. See troubleshooting tips in the FAQ. Don't just guess and throw parts at it.

The low hanging fruit would be filters, spark plugs, fuel pressure, cleaning the MAF sensor and throttle body.

you should help her take the car for an italian tune up once in a while and shake off the cobwebs.

Fuel doesn't age well.

Thats my exact intention next time I get my hands on it.  Probably will toss in some injection cleaner, check the air filter, and take it out in 2nd-3rd gear on the highway for 20-30 miles or more.
