2012 Toyota Sienna with 161,000 mi. Alternator and AC replaced within the past 2 years. Original water pump, radiator and pulleys. Rattling sound starts after the engine is warmed up and gets louder intermittently. Engine is not overheating.

I would make a guess that that is your timing belt tensioner ... I have a Honda Odyssey and it was recently replaced and it sounded very similar to your sound. I would use a Aisin Brand only OEM replacement.
