I have a 93 Ford E350 cargo van 5.8 with 256,0000 miles. It has a rough idle mostly in drive at a stop light, the whole van serges forward if i put it in neutral it stops. Iv replaced spark plugs wires and rotor, O2, map sensor, a couple of parts on the carb i cant remember the name now. What eles could be the problem, my friend thinks i have a vacuum leak thanks for your time.

It could be a vacuum leak. Pray that is the issue. I would also check the MAF sensor if you didn't check that.

try replacing the PCV valve (and inspect its hose too)
