I have a 2011 Honda Accord 120k miles, when i cold start it it makes a loud groaning noise. I've seen many Accords like this that make the same noise which means the VTC actuator is bad. What would happen if I don't get it fixed, any long term damage?

sometimes they can go years that way as long as the noise doesn't stay but just for curiosity take all the fan belts off and it's noise goes away you know it's something to fan belt drives like alternator
