I have an 06 Honda civic hybrid automatic transmission. 113000 miles. No codes. On cold mornings, idle is a little rough and continues as I reverse out of the driveway. After I shift to drive, it's smooth sailing from there. No issues when outside temperature is above 45F. Any ideas?

Well that's weird one try this first part that we don't have to back up if it does not act up there some kind of problem with reverse gear or the wiring in the transmission. But if it happens even then when you go in for words when it starts up and you're not backing up you might try tuning up the engine and doing my video make your car run better with a little spray cleaner
