I have 2005 Mercedes E320 with 213K miles. The car shifts very rough when cold. I noticed that adding a fuel system cleaner eliminates this issue while the cleaner is in the tank. Is this related to the fuel system and what could cause this?

Sure. If your engine doesn't Ron, right? It's going to make a shift. Weird. If adding that fuel cleaner actually does make it stop doing that. You need to work on a fuel injection system. Something's wrong with it
