I have 2008 GMC Sierra, 720,000 miles. Original Engine, I can’t figure out what making it idle weird. No check engine light should I live with it or try and find the problem?

What do you mean by weird? Is the idle RPM too low or too high? Excess vibration? Weird noises?
It surges, it’s almost got a million miles on it, hell I drive it like a stole it.
Start by cleaning your mass airflow sensor:
