1993 Toyota Pickup idle issues

Heya Scotty this is kinda a big one, I have a 1993 Toyota Pickup V6 4x4 3.0 VZE , manual transmission, with roughly 210k miles. This is my daily for work.

I bought this truck a year ago ( roughly around 189k miles )with a starting issue that’s since been resolved 

When I start and drive the truck long or short distances , after having the truck off for more than 10 min (like as if I were to go grocery shopping real quick) . The truck will start but then idle itself to death . Almost like drowning ? Large power loss and unless I can get it moving before it kills itself and just keep flooring it - I’ll be stuck waiting for it to kinda air out so to speak ? This problem happened only occasionally before... but now it’s everyday...

I guess to simplify the problem
When I cold start and drive , there is no problem.. but the moment I’ve had my


 
truck off for longer than like 10 min . I can’t drive it again until I’ve let it sit for an hour. If I try and drive it sooner , the truck starts then rapidly rough idles until dies. 

Now I’m no certified mechanic I’m just a 21yr old garage junkie but so far this is what I have replaced 

New starter

New vacuum hoses 

New battery 

New belts 

New Distributor cap & Rotor 

New gas pump filter 

New ignition coil and cylinder 

New spark plugs and wiring 

Even took some advice from online and cleaned out the area where the EGR valve enters the engine (gunk apparently builds up there , there WAS some but not a whole lot)

New Cold start injector and wiring harness

New gear oil in both differentials , transfer case and transmission. 
I change the engine oil every 3-5k never miss it 

Swapped MAF sensor with a same engine &year Toyota pickup (was in the junkyard) 

{(Unrelated) New fan shroud , new idler arm}

The previous owner of the truck had the radiator replaced 

New Coolant hoses 

I even took it to a specialist here in the 559 who was supposedly an expert in these trucks He had my truck all damn day and could not figure it out . Told me he’s stumped and that he has no clue (didn’t charge for the time either so I know he’s telling the truth)

The timing has been done and checked by same mechanic 

The butterfly valve on the throttle was corrected by same mechanic 

I’m a vehicle detailer by trade so although I can’t afford authentic parts from Toyota themselves , I have maintained this truck truck as clean as possible oil, fluids to steaming the carpets 
What am I missing ? I’ve poured so much money into this truck at this point I don’t wanna get rid of it . When it drives it drives solid- I absolutely love this truck 

recently...also in the cold morning even if I let the truck idle for 10-15 min to warm up , the truck has almost like a hesitation in 1st 2nd and 3rd, I slug forward until I reach around 3500 rpms then it skyrockets up. This lasts until I’ve driven some distance then it goes away. Relatively 1-2 miles from start  

honestly I have yet to find anyone with the same problem with me on the web so please if you could anything direction will help!

It sounds like a vacuum leak with one of the intake manifold gaskets combined with a small head gasket leak. You should try the propane enrichment method around the intake gaskets from a cold start. When you changed the spark plugs did you notice any white residue around the cathode? One last thing to check would be the catalytic converter. They start to get plugged up at the mileage and can affect idle speed after the car warms up. Before this the IACV is bringing the idle up to warm the engine faster. Afterwards the idle can drop too low with the clog. This will give off a really bad heat just underneath the car when you're driving. You can check that with a <20usd IR thermometer pointed at the tailpipe before and after the cat.
