My wife's dodge caravan jerks when shifting to overdrive and when you let off the gas and back on again. So I hooked up my Autel MX808 that @DayWalker told me about and drove the van while watching the live data on the transmission. When the torque converter locks it jerks and when you let off the gas it unlocks and jerks again when you get back on the gas and locks again. So I'm guessing torque converter is bad?

It most likely is a bad torque converter. Although it is possible that it is a bad transmission. Those were some of the weakest transmissions ever built.
