I am looking at test driving a 2021 Highlander. Looking into it and the newer Highlanders are getting a bad reputation for transmission issues.
I have inquired on this forum about this issue, but I have a follow-up question.
If I were to buy a out-of-warranty 2021 Highlander, is it worth it to try and purchase an extended warranty.

If you search the board you will find that the subject of aftermarket warranties have already been discussed to death here.
In general are rip-offs. They'll take your money and deny as many claims as possible. Even if a claim is honored they will only pay for the cheapest possible repair. Over time you will pay them thousands of dollars whether you wind up making a claim with them or not. You'd be better off just putting whatever they would charge you per month into a separate bank account to be used for repairs.
Note that one of the largest and most aggressively advertised extended warranty companies (Carshield) had to pay a $10 million fine to the FTC for deceptive advertising practices:
(From what I have seen despite this Carshield's ads have not changed much.)
The previous owner took the car in at 64k miles to have the "transmission" serviced and 12k miles later he put it up for sale.
