my dad has a 2012 toyota sienna with 70k miles.
we recently starting to hear a groaning noise, seems to be coming from the front
it only makes it when taking off from a full stop.
theres no codes, what could it be?
is there a worry or should i be concerned?

This could be a clutch problem, not a brake problem.
Check tranny fluid and user manual for fluid specs.
check under vehicle for anything out of place.
have dealer look at this asap, if its a bad clutch, you wouldnt want to get towed.

ive asked a mechanic friend, and he said it maybe a engine mounts?
but wearing out engine mounts wouldnt it be more of a knocking noise as the engine rattles and moves in the engine bay?
Clutch on an automatic minivan transmission...right.
So hows it going 2 weeks after i sent my advice?

Well, you should pin point the Jose. Pray it is not from the transmission.
That generation of Toyota's including the Camry and Sienna had more than few transmission problems with their 6 speed's I wonder what happened to Toyota.

Does the noise sound when you’re taking off and turning the wheel or in a straight line? Or both?
Is the sound affected by temperature?
sounds like a brake problem cause you said it only makes the noise during a stop.
remove the left front tire after you jack it up and use jack stands.
Check the brake pads, if less than 1/8 inch thickness then replace them.
Do same for other side.
Also check under vehicle and wht wheel wells for unusual things like branches or things out of place.

its making the noise when we accelerate from a stop, stoping has no noise, just when i accelerate from a stop.
Pull of the wheels and check the suspension components. Worn down tires can also make horrid noises.
Brake caliper not fully releasing. Brake pads not fully releasing due to seizing on slides, internally collapsed hose, caliper piston failure.
